Day Habilitation Services are designed to increase independence for people with disabilities through community inclusion, volunteering, activities and daily life skills.
Provide personal care to individuals with disabilities. Assist with goals and activities both in a classroom setting and out in the community. High School diploma or equivalent is required. NYS Driving License in good standing may be required in some programs. 1 year of job related experience preferred, specifically working with people with developmental disabilities or related field. Must be at least 18 years old and be able to lift a minimum of 50 pounds to apply for this position.
Skills Coordinators provide full support to consumers including; dining, lifting and direct personal care, development and implementation of activities, supervision/oversight of consumers to ensure that consumers are carrying out their goals as dictated by their care plan. They deliver and document services accurately within the assigned group. Skills Coordinators will promote independence, community inclusion, individualization, and productivity while respecting the consumer’s personal choice regarding lifestyle decisions and their ability to take informed risks.
Responsible for the implementation of the individualized day program plans as described in the consumer’s care plan with the purpose of developing each individual’s opportunities for self determination, independent activity, increased productivity and greater community inclusion. May function as a Team Leader for each consumer assigned and may be responsible for the direction of all staff assigned to that program area. High School diploma or equivalent is required. 1 year of job related experience is required, specifically working with people with developmental disabilities or related field. A minimum of an Associate’s degree or two completed years of college may be considered in lieu of the 1 year experience. Must be at least 18 years old to apply for this position. Must be able to lift a minimum of 50 pounds.
Resident Counselors at the Center work in our certified group homes throughout the Capital District supporting adults with developmental and intellectual disabilities including, cerebral palsy, Down syndrome, autism spectrum disorders, and other neurological impairments. They are responsible for assisting and promoting independence to people we support. Resident Counselors support people with life improvement skills in self-care and performance of housekeeping tasks. Resident Counselors explore new opportunities and support people with enriching social and recreational activities.
The Center is the largest provider of Residential Services in Upstate New York with over 50 programs. A High School Diploma or equivalent and a valid Driver’s License are preferred. Must be at least 18 years old.
The Center offers a variety of schedules including: Full-time, Part-time, Relief, Evenings, Weekends and Overnight hours.
Clinical Assistants at the Center work in our certified group homes throughout the Capital District supporting adults with developmental and intellectual disabilities including, cerebral palsy, Down syndrome, autism spectrum disorders, and other neurological impairments. Continuously maintains observation of the physical health and psychological health status of the people we support. The Clinical Assistant works cooperatively with the program nurse to administer orders and receives medications for the people we support according to agency policy and procedures.
A High School Diploma or equivalent and at least 1 year experience working with developmental disabilities required. A valid Driver’s License is required. Must be at least 18 years old.
Community Services Professional
Community Services Professionals (CSP’s) provide services and support to people with developmental and intellectual disabilities of all ages in a 1:1 setting in the community or in private homes for the Community Habilitation and In Home Respite departments. CSP’s provide caregiver’s a respite and provide people with learning opportunities in activities of daily living to achieve independence. CSP’s support people to develop skills in financial management, social relationships, household maintenance and accessing and participating in their communities.
Must have a High School Diploma or equivalent, a valid, clean, insurable NYS driver’s license and reliable transportation.
